MY STORY

Dr. Koushik Muthu Raja Mathivanan, M.D

From a small town in Salem to building care for every family.

01

Where It Began

I grew up in Salem, Tamil Nadu. Back then it was a small town, and the best medical care was not something you could simply walk in and get. It was a dream. When someone fell seriously ill, the whole family packed up and travelled to a big city. I watched them spend money they did not have on travel, lodging and treatment. I watched them stand in long lines that never seemed to end, often for days, just to be seen. Many came back with empty pockets. Some came back too late.

Salem has grown since then. The roads are wider and the town is busier. But for advanced lung care, many families still face the same long journeys, the same costs and the same waiting that I saw as a child. Those years never left me. Somewhere in my childhood a quiet promise took root: One day, good care would not depend on which town you were born in or how much money you had. Everything I have done since has been an attempt to keep that promise.

02

The Long Climb

Medicine taught me that hard work opens doors. Leadership taught me that some doors are locked from the inside. As I rose, becoming Deputy Medical Director at 33 and a Professor at 37, I learned that building something new is never only about skill. In every place I worked, I wanted to grow lung medicine into what patients truly deserved: modern procedures, dedicated services and trained teams. And in every place, I met walls.

Some walls were rigid hierarchies, where age and seniority mattered more than ideas. Some were endless red tape, where a simple patient-friendly change waited months for a signature. Some were people more interested in holding on to power than in what was best for the patient waiting outside. Breaking old systems is never easy. It costs sleep, it costs comfort and, at times, it costs goodwill. There were days I came home exhausted, wondering whether it was worth it.

Then I would remember a patient who could breathe again, someone who, just like the families from my hometown, had travelled far to Chennai looking for hope and the best care. And I would go back the next morning and push again. Slowly, the walls gave way. New services were built. Young doctors were trained. Patients who once had nowhere to go now had somewhere to turn.

03

Building My Own Path

There came a point when I understood that some things cannot be built inside someone else's walls. Leaving the comfort and security of a large institution was one of the hardest decisions of my life. But I wanted to create places where:

The patient comes first, not the paperwork, not the hierarchy and not the bill.

That is why I built KMR's Lung Clinix, a day care centre where patients get expert lung care without long hospital stays or heavy costs. It is why I am building the Chennai Institute of Pulmonology, and why I founded SPICE India and the International Academy of Thoracic Medicine: to share knowledge, train doctors, close the gap between city and village care, and speak up for patients in the rooms where policies are made. None of it came easily. All of it was worth it.

04

The People Who Hold Me Up

Behind every long day is a home that waits with patience and love. My Dad and Mom, and my entire family, have always been my strongest support. Through every setback and every challenge, they gave me the courage to keep pushing forward. Whatever I have built stands on their sacrifices and their faith in me.

In 2017, I married Dhivyashri. She has stood beside me through every risk, every late night and every uncertain step. When the road got hard, she was the reason I never walked it alone. In 2022, we were blessed with our child, Iniyazh R Koushik. Holding Iniyazh for the first time changed how I see every patient. Every person who walks into my clinic is someone's child, someone's parent and someone's whole world.

05

Why I Do This

I am still that small-town child who watched families travel far, spend everything and wait endlessly for care that should have been close to home. That is why I don't just treat patients. I build systems: centres, trained teams and networks of doctors, strong enough to carry good lung care out of the big cities and into smaller towns and villages.

No family should have to leave home, sell what they own or stand in a never-ending line just to breathe better. The titles, the training and the institutions are only tools. The goal has never changed: World-class lung care that every family can reach and afford, delivered with honesty, dignity and heart.

This is my story. And if you are reading it as a patient, it is now a part of yours too.
